All on 4 restores an entire arch of fixed teeth on just four implants, often without the need for bone grafting. It is the option chosen by people who have lost many teeth, or who live with a removable denture and want to bite down with confidence again.

In the All on 4 technique two implants are placed vertically at the front and two are tilted at the back. Tilting them makes use of the bone that is still available and avoids the areas where volume has been lost, which are usually the posterior ones. That is why many patients can be treated without a graft.
A single prosthetic structure carrying every tooth in the arch is then screwed onto the four implants. It is not a denture you take out: it is fixed, only the doctor removes it at a check-up, and you clean it daily with a brush and interdental brushes, like natural teeth.
Compared with a removable denture, the clearest change is in chewing and speech: the structure is anchored to the bone rather than resting on the gum, so it does not move while you eat or talk and it needs no adhesive.
How it works, step by step.

1. CT scan and planning
On arrival we take a CBCT scan to measure the available bone and set the angle and position of the four implants. This is the stage that determines whether All on 4 really is the right answer in your case, or whether six implants would be better.

2. Extractions, if needed
If any teeth in the arch cannot be saved, they are removed in the same session as the implant surgery, so you avoid a second surgical appointment and a further trip.

3. Placing the four implants
The surgery is carried out under local anaesthetic, with conscious sedation if you prefer, one arch at a time. The two posterior implants are tilted to increase the anchorage surface in the bone that is available.

4. Fixed temporary prosthesis
In the days immediately after, a fixed temporary prosthesis is screwed into place so you fly home with teeth. During healing you follow a soft diet, according to the surgeon's instructions.

5. Final prosthesis
After healing and osseointegration you return to Tirana for impressions and the fitting of the final prosthesis, made in the agreed materials and refined for aesthetics, occlusion and speech.
How many days and how many trips
All on 4 needs two trips: the first covers the surgery and the fixed temporary, the second the final prosthesis. The first stay is longer than for a single implant because it includes surgery across the whole arch and the post-operative check.
