All on 6 supports a full fixed arch on six implants rather than four. Spreading the load across more anchorage points is indicated when the available bone allows it, and where more chewing stability is wanted — particularly at the back of the mouth.

The logic of All on 6 is the same as All on 4 — a single prosthetic structure screwed onto implants, not a removable denture — but with two more points of support. The six implants are distributed along the arch so that the posterior sections, which take the greatest forces during chewing, are supported too.

It is not automatically the better option, though: it needs more available bone, because each implant requires its own volume of anchorage. That is why the choice between four and six implants is not made before the CT scan but after it, based on what the scan actually shows.

Where the conditions are there, spreading the load further is an advantage over the long term — particularly for people with strong chewing musculature or a history of bruxism — and in some cases it allows more extensive prosthetic structures.

How it works, step by step.

  1. 01

    1. CT scan and case study

    The CBCT scan measures bone volume and density along the whole arch. It is the examination that establishes whether the conditions for six implants exist and where to position them, or whether four would be the more prudent choice.

  2. 02

    2. Treatment plan and final quote

    The surgeon takes you through the plan: implant positions, any extractions, the type of temporary and final prosthesis, timings and costs. The quote is confirmed at this stage, after the examination.

  3. 03

    3. Placing the six implants

    The surgery is carried out under local anaesthetic, with conscious sedation if you prefer, one arch at a time. The implants are placed according to the digital planning done on the CT scan.

  4. 04

    4. Fixed temporary prosthesis

    In the days that follow, a fixed temporary prosthesis is screwed into place and you fly home with it. Through the months of osseointegration you follow a soft diet, as instructed.

  5. 05

    5. Final prosthesis

    On the second trip we take impressions and fit the final structure, refined for occlusion, aesthetics and speech, and checked across several appointments before it is handed over.

How many days and how many trips

As with All on 4, two trips are needed, separated by the osseointegration period. The first stay covers the CT scan, surgery, temporary and post-operative check; the second is for impressions, try-ins and fitting the final prosthesis.
